The sports sign-up registration form will open as of October 1st and is located online in the Genesis Parent Portal under the “Forms” tab. 

You must complete an online sign-up form for each child in your family who is signing up for a FALL sport and acknowledge your receipt of the concussion, sudden cardiac, opioid and eye injury fact sheets.


In order for your child to participate in a sport at EHMS, you and/or your physician must complete three forms required by the State of New Jersey: a Health History Form, a Physical Exam Form and a Sports Permission/Emergency Information Form. All of these forms are located in the Hoppock health office or can be downloaded online via the sign-up registration form and/or the school website below. The forms must be completed by November 1, 2018 in order to provide the school with adequate time to obtain final medical approval from the school doctor prior to the start of sports’ tryouts/practices.

Health History - MUST BE COMPLETED ONCE PER CALENDAR YEAR by you and reviewed by your health practitioner (physician, physician assistant or nurse practitioner)

If the health questionnaire was completed more than 90 days prior to the first day of practice, a health history update needs to be completed for each season

Physician Evaluation must be completed by your health practitioner within 365 days of the first practice session (once per calendar year).

Sports Permission – At sign up emergency information for coaches must be completed online. This form is required for each sports season. 

Only those students with all medical documentation completed and approved by the school medical examiner at the start of the school year will be allowed to participate in any sports practices or competition.
